How's your case management process? Mark your calendar now to join us along with other practitioners and experts this fall at the Hilton Hotel San Diego Mission Valley for Evidence-based Programs & Supports Workshops & Conference for Case Management & Care Coordination.

This program is designed for the clinical and non-clinical practitioner who functions anywhere in the system. It focuses on micro and macro strategies that are clinical, non-clinical as well as system-wide considerations for implementation and practice. Participants will be presented with specific skills and approaches to help them gain understanding of the way in which their specific activity fits in the big picture with regard to service delivery and outcomes.

WHAT TO EXPECT

We kick off on Day-1 with 2 full-day workshops, 1) focused on the Core Skills for Evidence-Based Case Management, and 2) How to Become An Evidence-Based Case Management Organization.

On Day-2 we open with 2 general sessions reviewing current and coming trends in case management and dig deeper into how health and wellness is currently being integrated and what you and your team can do to make the inevitable transition. Day-2 then splits into a choice of 2-Tracks you may choose from. Track-A reviews “rubber-meets-the-road” approaches. Track-B on the other hand is very focused on the reviewing the challenges of Trauma Informed Care implications both at the agency, client and staff levels.

We continue on Day-3 with again 2 Tracks to choose from. Track-A once again continues the focus on client-facing activities, Person Centeredness and Treatment and Services Planning. Track-B takes on Ethical issues for case management followed with a discussion on Supervision. 2 general sessions round out the program. The first looks at the role of the Liaison as the go-between advocating for and working to remove obstacles clients and various agencies may encounter as they each work to successfully navigate.

Of course, no Joyfields Institute program is complete without the Action Planning piece which is where participants spend some time organizing an action plan for implementing what they have learned upon their return.

There is no question this program is chuck full of sessions case management and are coordination personnel everywhere will benefit from regardless who your client population is, mental health, housing, workforce development, youth, adult, justice involved or corrections.

Motivational Interviewing Skills – This year for the first time, we are offering an optional MI course for those interested to attend.  This is a 2-Day program offering a full 13 CE's. It available FREE to registered attendees to the Case Management program. Alternately you may register separately for the 2-day course. Just let someone at our office know prior to arriving on-site.

At the conclusion of the program, attendees should be in a position to;

  • Describe evidence-based approaches for case management and understand current and upcoming trends

  • Integrate motivational interviewing (MI) strategies, cognitive behavioral (CBT) skills, person centered planning and trauma informed care program approaches in their practice

Its my hope that you and your team will find time to join us for this program Every social and human services agency cannot afford to miss. Bring your team, stay together, or split into various tracks and get everything your agency needs to implement, enhance or round out your evidence based and strength centered efforts.

If I can help in anyway to make your participation possible and more enjoyable, please do not hesitate to let us know.

See you in San Diego!

I liked that we were provided with specific tools (questionnaires and assessments) to utilize with our agencies. I am in strong agreement that we need to focus on internal processes and on staff outcomes as part of an evidence-based approach and I think it's great that the workshop highlighted this throughout “
